A 50-year-old non-alcoholic female presents with acute confusion and vomiting. History from her husband reveals that she has taken several pain killers for an acute gouty attack over past few days.
Vital signs are unremarkable. Blood investigation reveals Hb 11 g/dl, WBC 9 109/L, Platelet 390,000 mL, ALT 1321 U/L, AST 800 U/L, Alkaline phosphatase 159 IU/L, Bilirubin 23 mg/dL, INR 3.1 and creatinine 250 mmol/L.
