Atrial Fibrillation Case 1

A 78-year-old man with a history of diabetes attends the ED complaining of palpitations. There has been no chest pain and no precipitants.

The episode began abruptly 3 days ago. He is normally fit and well, is a non smoker and he is not on any medication.

On examination:

  • Heart rate varies between 128 bpm and 147 bpm
  • BP = 115/80 mmHg
  • Heart sounds are normal with no murmurs
  • Chest is clear to auscultation
  • Clinically euthyroid
  • Apyrexial
