A 75-year-old man presents with a 3-day history of lethargy, dizziness and general myalgia. He also complains of blurred vision and muscular weakness. His current symptoms were preceded by a week of diarrhoea and vomiting.
He has a past medical history of ischaemic heart disease and hypertension and is on aspirin, atenolol and ramipril. On examination, he is dehydrated, his pulse is 50 and blood pressure 95/60. He is alert and orientated. Intravenous atropine has been ineffective.
