A 73-year-old woman presents with a collapse preceded by sudden onset dizziness and chest ache. She has had similar less severe episodes over the last few months. She had an MI 3 yrs ago. On arrival her symptoms were less marked and her chest pain had resolved. On examination, her pulse varied from 45 to 100 and BP was 127/80.
