A 40-year-old presented to the ED with a history of urinary retention for 12 hours and pain at the tip of their penis. They have been managing to pass dribbles but nothing more. There is no penile discharge, and they last had sexual intercourse six months ago. There has been no penile trauma. There is no history of back pain, or lower limb neurology, bowels are opening normally, and they have only mild abdominal pain.
This has never happened to them before. They are normally fit and well with no allergies.
On examination they have normal observations with a firm lower abdomen felt on palpation, suggesting a full bladder.
