An 82-year-old female presents to the ED with respiratory difficulty. She is known to have MG with no thymoma on a previous CT scan. Her neighbour, who called for the ambulance, noticed she has left sided facial weakness today and is ‘wheezy’. Paramedics have treated her with a salbutamol nebuliser. They inform you that she has a positive FAST test.
On arrival in the ED:
- She is conscious
- She is afebrile
- PR 95
- BP 160/95
- RR 24
- O2 sat 92% on room air
- She has an inspiratory stridor and a hoarse voice
- On chest auscultation, she has transmitted upper airway noises
- She has a right-sided ptosis, left-sided facial weakness and no other obvious neurological deficit
- Her stridor seems to improve when reassured
