A 36-year-old female, in her 36th week of pregnancy, presents to the ED. She reports feeling increasingly tired over the last few weeks and that her ankles have been swollen. However, today she suddenly developed breathlessness whilst reading a book of baby names.

Her observations are as follows:

• Respiratory rate 28
• Pulse 102bpm
• BP 90/60 mmHg
• SpO₂ is 92% on room air
• Temperature 37.3 °C

She looks a little pale and has bilateral ankle oedema to the mid calf level.
