A 27-year-old female presents to the emergency department (ED) with one day history of sudden onset of moderate to severe intensity headache, vomiting, diplopia and abdominal discomfort.

The patient doesnt report any rash, neck pain or photophobia. No urine or bowel related symptoms have been identified. Her past medical history includes migraine. She delivered twin babies 3 weeks ago.

On examination she looks dehydrated with a supple neck with the following observations:

SPO2- 97% Room Air
R/R- 22
BP- 80/50
Temperature – 37.8 C
GCS- 15/15
