A 25-year-old male with no significant past medical history attends Emergency Department (ED) with chest pain. He has a 3 day history of a cough with green sputum but no shortness of breath. His chest pain was dull and worse on movement but not with breathing.
Observations were pulse 88/min, BP 135/70, SaO2 99% on air, RR 14, apyrexial. Heart sounds normal and chest clear on examination. Chest x-ray showed clear lung fields with normal heart size. Initial bloods WCC 15, CRP 172, U&E normal. ECG was done (shown below) and troponin level 528.
