Breath of Death?

A 61-year-old female with a history of sarcoidosis is admitted to your department in acute respiratory distress.

She is usually on home oxygen. She had been discharged from hospital 1 day previously after having a tension pneumothorax drained. Initial observations were that she was unable to speak in sentences, talking in gasping words only, SpO2 88% on 15L NRB O2, Respiratory rate 40, no air entry right side of chest, hyper-resonant to percussion with surgical emphysema, healing right sided chest drain scar. BP 231/111, heart rate 116bpm, GCS 15.
