A tall, thin 19-year-old man presents with sudden onset of sharp, stabbing bilateral chest pain, worse on inspiration and most severe under the left scapula. This is associated with breathlessness on exertion.

On examination:

  • Comfortable at rest
  • Equal bilateral air entry
  • Respiratory rate: 20 breaths/minute
  • Oxygen saturation: 98% on room air
  • Pulse: 88 beats/minute and regular
  • Blood pressure: 130/80 mmHg
  • Temperature: 37.0 degrees Celsius
