A Real Pain in the Neck

A 67-year-old man, who had a background of diabetes, presents with pain on the left side of his neck, lower back pain and numbness in his left arm.  

He had attended 1 week previously with haematuria having sustained local penile trauma having struck himself whilst attending a gym class.  He underwent a cystoscopy following this. 

On examination he was febrile with a temperature of 38.6 degrees, heart rate was 76 bpm, BP 125/67 and had grade 4/5 power in the left arm, 5/5 right arm and reduced sensation to light touch on the left side.
