A 63-year-old male, who speaks only Hindi, is brought to the Emergency Department (ED) by his son due to acute confusion and aggressive behaviour. His son explains to you that he had ran out of his house and damaged public property.

On examination, the patient exhibits involuntary movements of the face and upper limbs. His son reports that these movements have been progressively worsening over the past year, but that the confusion has only started recently.

The patient was independent a year ago, but he began deteriorating after the death of a close friend. There is no significant past medical history or medication use. The son mentions that his fathers siblings all died in their 60s, but he is unaware of the causes.
