Acute Severe Wheeze

A 5-year-old girl presents with acute shortness of breath and wheeze. She has a past history of asthma with one hospital admission and course of steroids in the past year.

On examination she has marked intercostal recession, respiratory rate 55 and bilateral expiratory wheeze. Saturations are 94% on an oxygen driven nebuliser.

She is just finishing her first 2.5mg salbutamol nebuliser. Her mum has given her 20mg of prednisolone this morning.
