An Unexpected Guest

A 20-year-old male presents to the Emergency Department (ED) with intermittent central chest pain for the last 6 months. He is a smoker but has no significant past medical or surgical history. His main concern is that he is having a heart attack.

Initial observations are normal, and he has an unremarkable clinical examination.

ECG, venous blood gas and lab bloods including troponin are also all normal.
