Leaching cells

A 62-year-old female with lymphoma who had her last chemotherapy 4 days ago presented to the Emergency Department (ED) with palpitations, weakness and generalised lethargy. She was apyrexial.

On examination her observations were within normal limits.

Cardiovascular exam revealed a tachycardic thready pulse with cold peripheries. Her respiratory exam was unremarkable.

Her initial blood gas revealed:

pH 7.31,
HCO3 20 mmol/ L,
Lac 3 mmol/L,
K 6.8 mmol/L,
Na 132 mmol/L,
Cl 98 mmol/L,
Ca 0.93 mmol/L
Uric acid levels were 800 mol/L
Phosphate 2.6 mmol/L
