Paediatric Inflammatory Multisystem Syndrome (PIMS-TS)

A 9-year-old boy presents to your emergency department after feeling unwell, with a fever and painful swollen feet gradually worsening over the past 5 days. 

His parents had both tested positive for COVID-19 three weeks ago when they had a cough, fever and generalised muscle aches. The family self-isolated and both his parents made a full recovery. His 3-year-old sister remains well. 

He is assessed by the paediatric emergency medicine resuscitation team, intravenous access is secured and an intravenous fluid bolus is given. The working diagnosis is paediatric inflammatory multisystem syndrome temporally associated with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (PIMS-TS).
