A 79-year-old man presents to the Emergency Department (ED) with persistent right inguinal pain and intermittent left flank pain over the past week. His medical history was of no significance and the clinical examination revealed moderate tenderness over the right inguinal canal with a protruding mass within the canal. The laboratory workup was unremarkable.

An abdomen/pelvis CT scan reveals moderate diverticular disease of the sigmoid colon with mild fat stranding and an uncommon diagnosis on the right side.
